At Urology & Oncology Specialists, PC, we respect the privacy and confidentiality of your personal health information. This notice describes our legal duties and privacy practices. This notice applies to uses and disclosures we may make of all health information whether created or received by us. The complete notice will be provided to you at your first appointment and a copy is available for review at each office.
Medical Records
Medical records are maintained in strict confidence. Information will not be released without a properly executed Release of Records, signed and dated by the patient or the patient's legally appointed representative. Please remember that requests for medical records take time to process (at least 24 hours) and plan accordingly.
